Generation Check Individual Registration Form

First Coast Regional leaders participated in Reality Check First Coast on May 21, 2009. This brochure will give you an idea of the scope of that event. It is important to realize that the average age of participants at Reality Check First Coast was over 40. Their work, experience and knowledge are necessary to our communities’ success, but since we are visioning for a 50 year period, it is vital to engage youth in thinking about the future of the First Coast. We want to begin to encourage active participation within the community by those who are tomorrow’s leaders. We want you to join our efforts to create a vision for the future of the First Coast Region. If you are between the ages of 13 and 17, you are required to bring with you to the event Waiver of Liability and Hold Harmless Agreement and Emergency Medical Authorization forms signed by a parent or legal guardian.
Community Service/Volunteer Hours
We encourage the participants in Generation Check to document their participation. The participant may either bring a community service /volunteer form from their school or organization, to be signed by the Facilitator at the event, or may get one from the Facilitator.
The Regional Community Institute of Northeast Florida, Inc., which was created by the Northeast Florida Regional Council to work on a First Coast Vision, and Region First 2060, the partnership that put on Reality Check First Coast last May, have teamed up the Girl Scouts of Gateway Council, Inc. to host:

This will be a free regional visioning exercise, exclusively for young people. Lunch will be provided to all participants. If you are between the ages of 13 and 22 please register below.
